Case Management Nurse - Case Manager

OhioHealth Hospice professionals are part of a multidisciplinary team – including social workers, therapists and volunteers. We’ll provide guidance, medical care, personal support and information. In addition to coordinating care with your doctor, we’ll work closely with you and your family – mindful that you’re in control of your care. This position is responsible for identifying and coordinating patient/family care needs to provide palliative and supportive care to the terminally ill and their families. He/She utilizes teaching, assessment and intervention skills to maximize quality of life to patient and family in all settings of hospice care. This position directs the work of home hospice aides/homemakers and/or patient care volunteers and is responsible for promoting the hospice philosophy within the organization and community. The hospice registered nurse is committed to the philosophy, goals, and objectives of the hospice concept. This position is primarily for an admission nurse, but also addressing on call needs and symptom management. Will rotate on call with team nurses. Candidates must provide stellar customer service and must be a caring, passionate, compassionate team member and provider.
